Treatment, payment, and health care operations

To prevent a serious threat to health or safety
Other uses or disclosures will be made only with your written authorization.

Request access to or a copy of your records
Request corrections to your records
Request restrictions on certain uses or disclosures
Receive a copy of this Notice
Requests must be submitted in writing.
Telehealth
Electronic communication and telehealth services carry some privacy risks. Reasonable safeguards are used to protect your information.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Fa’akasi Family Counseling or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
